What should I do if I am injured at work?
ALWAYS report the injury using the procedure provided by your employer if they have one. Do not think that you are helping yourself or your employer by not making a report. Late reporting NEVER helps the injured worker.
If there is any doubt about whether or not your employer reported the injury you should use a form provided by the State of Michigan. We will mail you a form if requested at no charge.
This form should be mailed to the State at the address on the form & your employer.
